The Role of Density in Achieving Volume and Weight Symmetry in Breast Reconstruction

背景 在单侧乳房重建中实现体积和重量的对称性时,了解组织和植入物的密度至关重要。因此,本研究旨在测定并比较腹部组织标本、乳房组织标本以及第五代乳房植入物的密度。

方法 对接受隆胸或腹部整形手术的 61 名患者的 31 个乳房组织和 30 个腹部组织样本以及 5 种不同的第五代乳房植入物进行了检查。采用水置换法计算密度(g/mL)。

结果 乳腺组织平均标本密度为 0.94±0.02 g/mL,腹部组织平均标本密度为 0.94±0.02 g/mL,差异无统计学意义(p=0.230)。乳腺组织密度随年龄增长而显著(p=0.04)下降,而腹部组织密度没有显著变化。本文提供了一个回归方程来计算经年龄校正的乳腺组织密度(乳腺密度 [g/mL] = 0.975-0.0007 * 年龄)。乳腺组织密度与体重指数、既往妊娠史或母乳喂养史无关。乳房植入物的密度范围为 0.76 至 1.03 g/mL,与乳腺组织密度有显著差异(-0.19 g/mL [-19.8%] 至 +0.09 g/mL [+9.58%];p ≤ 0.001)。

结论 我们的研究结果支持腹部穿支皮瓣适合在单侧自体乳房重建中实现体积和重量对称。腹部皮瓣体积可与乳房切除术重量一一对应地得出。此外,鉴于不同品牌间存在显著的密度差异,在进行大乳房的单侧植入物重建时,应考虑可能造成重量不平衡的潜在风险。
