Clinical Outcome in Patients with Large Sinonasal Tumors with Intracranial Extension

目的 鼻窦恶性肿瘤扩散至额颅底是一种罕见且具有挑战性的病症。对于特定病例,采用额底开颅术和鼻内镜手术重建前颅底,然后进行辅助放射治疗的联合手术是首选治疗策略。该人群的发病率和死亡率很高。我们旨在将我们的经验补充到现有文献中。

设计 我们对2010年至2021年间荷兰一家三级大学转诊医院的长期临床结果进行了一项回顾性横断面单中心研究。进行了描述性统计和频率分布分析。

参与者 从电子病历中提取了18名连续患者的患者、肿瘤、治疗、并发症和生存特征。

主要结果指标 主要结果指标为无进展生存期、总生存期和并发症发生率。

结果 共纳入 18 名连续患者,平均年龄 61 (SD ± 10) 岁(范围 38-80 岁);10 名男性和 8 名女性。14 名(77%)患者实现了全切除。11 名(61%)患者接受了局部放疗,1 名(5%)患者接受了化疗,3 名(17%)患者接受了化疗和放疗联合治疗。平均随访时间为 49 个月(范围 3 – 138 个月)。3 名(17%)患者因术后并发症在医院死亡。6 名(33%)患者在随访期间因病情进展死亡。平均无进展生存期为 47 个月(范围 0 – 113 个月)。

结论 总之,本组鼻腔鼻窦大型肿瘤患者的总生存率为 50%。疾病进展严重影响生存率。5 名 (28%) 患者出现手术并发症。放射治疗并发症发生率较高。放射性坏死是两名患者的严重并发症,可用大剂量类固醇治疗。
