无论是保守治疗还是手术治疗，舟骨骨折及不愈合目前仍是难题。舟骨是维持近端腕骨列长度和排列的支柱，不稳定的舟骨骨折或不愈合会使远端舟骨碎片屈曲，其余部分（近端舟骨、月骨、三角骨）伸长，腕关节不能再保持长度，造成背侧插入节段不稳定（DISI）畸形，即腕骨不稳定。一旦发生DISI畸形，从桡骨茎突开始，到桡舟骨、舟头关节，都会发生退行性改变，称为舟骨不愈合晚期塌陷。另外，舟骨近端的血供也相当不稳定，有时舟骨腰部或近端骨折后，可发生近端坏死。这就是为什么通过计算机断层扫描或核磁共振成像对舟骨骨折进行早期诊断非常重要，并且对于不稳定的舟骨骨折/不愈合，尤其是在成年人中，适当的手术治疗算法是必要的。
本期的”Special review“是 Samade 博士和 Awan 博士撰写的“舟骨骨折的手术治疗：管理建议”。他们对近期有关舟骨骨折/不愈合手术治疗的文献进行了全面综述，并试图编制出对读者有益的最新治疗建议算法。本期还包括几篇有趣的腕关节论文，例如远端桡骨骨折移位的风险因素、三角纤维软骨复合体撕裂伴有尺侧腕伸肌问题、掌指关节关节镜检查，另外还有新兴技术论文、调查和荟萃分析、有趣的病例报告，不要错过它们。
