Revealing Melatonin’s Mysteries: Receptors, Signaling Pathways, and Therapeutics Applications

Axillary Node Metastases of Medullary Thyroid Cancer: A Hallmark of Terminal Disease

目前对甲状腺髓样癌 (MTC) 腋窝淋巴结转移的了解甚少。为了解决这个问题,对有和没有腋窝淋巴结转移的 MTC 患者进行了比较研究。在 1215 名连续 MTC 患者中,482 名患者为淋巴结阴性 MTC,733 名患者为淋巴结阳性 MTC。733 例淋巴结阳性 MTC 患者中,4 例患者(0.5%)出现腋窝淋巴结转移,且均为同侧。腋窝淋巴结转移患者切除的淋巴结转移灶数量是正常患者的 5.7-6.9 倍,在作者所在机构(中位数为 34.5 vs. 5 个转移灶;p=0.011)和总体(中位数为 57 vs. 10 个转移灶;p=0.013)均如此,且发生远处转移的几率更高(3/4 vs. 178/729 名患者,或 75 vs. 24%;p=0.049),具体为骨转移(2/4 vs. 67/729 名患者,或 50 vs. 9%;p=0.046)和脑转移(1/4 vs. 4/729 名患者,或 25 vs. 0.5%;p=0.027),且死于癌症特异性死亡的几率更高(3/4 vs. 52/729 名患者,或 75 vs. 14%; p=0.005)。总体而言,腋窝淋巴结转移患者同侧颈部淋巴结转移数(中位数 11 vs. 3;p=0.021)和同侧中央颈部淋巴结转移数(中位数 8 vs. 1;p=0.079)均比无腋窝淋巴结转移患者多 4-8 倍。有腋窝淋巴结转移的 MTC 患者的癌症特异性生存期明显短于无腋窝淋巴结转移的 MTC 患者(平均 41 vs. 224 个月;plog-rank<0.001)。这些结果表明,有腋窝淋巴结转移的 MTC 患者转移性播散广泛,生存率低。
