冷冻消融(CBA)作为内镜下治疗巴雷特食管(BE)的射频消融(RFA)替代方案是否有效?Sachdeva等人通过回顾性研究对比了RFA组(n=610)与CBA组(n=71)患者肠上皮化生完全缓解后的表现,两组中位随访时间均约4年。在排除胃食管连接处肠上皮化生后,研究发现两组“任何复发”风险相当(RFA vs. CBA风险比[HR] 1.18,95%置信区间[CI] 0.61–2.30;P=0.62),倾向评分匹配分析(各54例)也支持这一结论。基线最大BE长度是唯一与“任何复发”(HR 1.07;P<0.001)或“异型增生复发”(HR 1.11;P=0.001)风险升高相关的因素。
当内镜逆行胰胆管造影术(ERCP)难以实施时,内镜超声引导下经腔内吻合金属支架(LAMS)行胆总管十二指肠吻合术(EUS-CDS)是胆道引流的备选方案,但存在技术失败风险。Beunon等人对比了100例失败病例与196例成功病例,发现技术失败的风险因素包括胆总管直径≤15mm、十二指肠狭窄、跨导丝技术应用及术者经验≤10例。内镜补救成功率达77%,而EUS-CDS失败患者的30天死亡率达12%。

Background: Endoscopic eradication therapy (EET) is recommended for the management of dysplastic Barrett’s esophagus (BE) and intramucosal adenocarcinoma. Both radiofrequency (RFA) and cryoballoon ablation (CBA) can induce complete remission of intestinal metaplasia (CRIM). We aimed to compare long-term durability following CRIM in patients treated with RFA vs. CBA.

Endoscopic bariatric and metabolic therapies (EBMTs) are increasingly recognized as valuable tools for managing obesity and related metabolic disorders. As the prevalence of obesity continues to rise globally, there is a growing demand for effective, safe, and less invasive treatment options. This review provides a comprehensive summary of the available EBMTs, including both stomach-targeted and small-bowel-targeted devices. The review details the various devices, outlines the techniques for their proper use, and discusses their indications. It also presents data on their efficacy and safety, and the management of adverse events and weight regain, as well as anticipated future developments. The paper emphasizes the necessity for additional high quality randomized controlled trials and long-term outcome data to better define the role of these therapies within obesity treatment protocols.
